New patients are required to complete this form prior to the first visit. It collects dental and medical history information that will assist Dr. Fleming in helping you make wise dental care decisions. To avoid needing to fill it out at the office, please print and complete this form, and bring it with you to your first visit.

Insurance and Payments
We accept most dental insurance plans, as long as you are not required to see a specific dentist to have coverage. We are “in-network” with Delta Dental. If you are unsure whether we take your insurance, please fax us (919-479-1841) a copy of the front and back of their dental card, and we will verify your insurance plan.
Our payment options are as follows: Cash/Check, Visa, MasterCard, Discover and we accept CareCredit. CareCredit is a long-term financing option that has no-interest plans offered.
